- First and Last Name of each traveler according to passport
- The gender and birthdate for each traveler 
- The room category you're interested in booking
- The dates you'd like to travel
- Yes/No on travel insurance -- make sure to note which type 1) Travel Credit Refund or 2) Cash Back Refund
- Any other questions you may have pertaining to travel plans or the hotel
